SELF-INSPECTION – Serious incident: wall collapsed!

Due to inadequate SELF-INSPECTION, we had a serious incident this week in which a wall toppled over at a construction site. The wall was standing on a truck bed that had been parked at the site without the driver having performed a self-inspection. At some point during loading, transport, or parking, the wall had shifted backward far enough that the brackets on the gallows could no longer hold the wall in place laterally.

When the load restraint was released during assembly, the wall toppled over onto the person who had released it; incredibly, that person escaped by a matter of millimeters.

This would never have happened if the load had been inspected when the trailer was unloaded. In that case, the issue would have been reported, and appropriate action would have been taken based on the situation.
